Below is a list of requirements needed in order to complete the contracting processing.
1. Licensing Questionnaire
2. Carrier Appointment Application
3. Electronic Funds Transfer (void check included)
4. Copy of current E&O Coverage
5. Agent/Agency State Insurance License
- Annuity – state specific 4-hour completion certificate
- Life Insurance – anti-money laundering certificate
- Long-Term Care Insurance – CE certificates for initial 8-hour and current 4-hour completion certificate
